This presentation will provide an overview of a health insurance recovery action on behalf of health care providers. The presenters will discuss the various state and federal law claims and defenses implicated by such an action, potential counter-claims by insurers, the medical necessity condition to coverage, proper application of plan benefit schedules and reimbursement methodologies, and how to marshal in the evidence.
